Terms of Participation: I, a parent/guardian of the participant named, do hereby state to the best of my knowledge there are no physical or other health-related conditions which will interfere with my child’s participation unless disclosed above.

I understand and acknowledge that Fastpitch softball and such recreational activities have inherent risks, dangers, and hazards both foreseeable and unforeseeable that may result in injury, illness, death, or property damage. On behalf of myself, my family, agents and contractors, I release and agree to hold harmless eXpress Softball, its sponsors, its volunteer coaches, managers, club officers, and directors from all claims, actions, or losses related hereto. eXpress Softball will assume no liability for injury or damage arising from the results of my child’s participation, unless due to willful fault for gross negligence on its part.

I agree my child will be covered by National Casualty Company & Nationwide Life Insurance Company, a policy purchased by eXpress Softball.

I additionally authorize any adult volunteer of eXpress Softball to obtain any necessary medical treatment for my child on my behalf, in case of an emergency, where I am not present and with the understanding that I will be notified as soon as possible.
